Skilled Registered Nurse

Skilled nursing services are provided by specially trained  registered nurse and licensed practical nurse. services are prescribed by s doctor on an intermittent basis. It includes such things as dressing change, intravenous therapy, catheter care, administration of medications, and teaching of patient and family how to care for the patient in the home.

  • Advanced Wound Care
  • COPD Management
  • Ostomy Care
  • CHF Management
  • Diabetic Care & Management
  • Colostomy Care
  • Managing Symptoms of Chronic Condition
  • Collecting Samples for Lab Test & Monitoring


Rehabilitation Services

Skilled Registered Nurse

Skilled Registered Nurse

Rehabilitation services include physical medicine. They are provided on as-needed basis in the home for comfort or goals of care. 


Services Triage Home Health offers but not limited to: 

  • Orthopedic & Neurological Rehab
  • Balance and Fall Prevention
  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Pain Management
  • Patient and caregiver education


  • PT in the home to reduce discomfort during activities ambulation and transfers 
  • OT Occupational Therapy may involve functional training such as appropriate techniques for lower and upper body dressing, correctly performing activities of daily living (ADLs) like bathing, dressing, and grooming, and inspecting the home for safety issues or risks.
  • ST Speech Therapy may involve teaching proper eating techniques to prevent dysphagia or exercises to overcome speech impediments.

Medical Social Services

The social services staff provides social casework services to patients as needed to augment other services of the agency Services include the evaluation of social functioning, particularly as it relates to medical problems, and initiation and follow up to other community resources as applicable.

  • Counseling
  • Patient and caregiver education

Certified Home Health Aide

Certified Home health aides work under the supervision of registered nurse. Under Medicare, a home health aide may stay in the home for a limited time or for a temporary visit only and is authorized to  provide personal care only.
Services primarily consist offer: 

  • assistance with personal care. A home health aide may give bath, give oral care foot career, shampoo, shave, skin care, 
  • Assist to change linens. 
  • Meal preparations

3. Improve access - help connect individuals to community health care services, education available resources, ensure the organization's offerings align with local priorities

4. Foster relationships: they cultivate strong relationships with community leaders, organizations and resident to promote collaboration and improve health outcomes.

We have Volunteers who willingly offer their time, skills, and services to support an organization, cause, or community without receiving financial compensation. Their contributions are driven by a desire to make a positive impact, gain experience, or support a mission they believe in. Volunteers play a vital role in our company, including healthcare, education, and community development, often helping to enhance the reach and effectiveness of programs and services.

Patient must be homebound in order to qualify for homehealth

  •  You have trouble leaving your home without help (like using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an injury or illness.

  • Leaving your home isn't recommended because of your condition AND
  • Your normally unable to leave your home and leaving home is a major effort. 

Note: You may leave your home for medical treatment or short, infrequent absences for non-medical reasons; like attending religious services. You can still get home health care of you attend adult day care.

What the home health staff do?

Patient must be homebound in order to qualify for home health services.

  • Check what you're drinking
  • Check your blood pressure, temperature, heart rate, and breathing.
  • Check that your taking your prescription and other drugs and any treatments correctly.
  • Ask if you're having a pain. 
  • Check your safety in the home. 
  • Teach you about your care so you can take care of yourself. 
  • Coordinate your care. This means must communicate regularly with you, your doctor, and anyone else who gives you' care.
