Restore Health
Help patient recover from an acute illness, injury, or surgery by providing skilled nursing, evidence- based rehabilitation at home that improve function and promote healing.
Improve or Maintain Function
- Provide therapy services to regain or maintain mobility, strength, communication, swallowing, and independence.
- Services include: RN, LPN, PT, OT, SLP, MSW, HHA
Care can begin within 24 hours. An on-call nurse is available on weekends and after hours.
Prevent Hospital Readmissions
- Monitor patient changes and their condition.
- Identify complications early and intervene promptly.
- Reduce the risk of ER visits or hospitalizations.
Teach Patients and Caregivers
- Educate patients and caregivers on managing conditions safely at home.
- Medication & disease management, wound care, fall prevention, infection control, and DME use.
- Social Services help access to community resources and referrals.
- Care coordination, counseling and psychosocial assessment.

How do I qualify for home health services?
Eligibility for home health care requires that patient be homebound.
The patient has a normal inability to leave home, and doing so requires a considerable, taxing effort.
The requested home health services must be medically necessary for treatment.
The care plan must be reasonable and proportional to the patient's specific clinical needs.
Referrals must be ordered by a physician and regularly monitored by a Primary Care Physician.
Some common signs that may indicate a home health need:
- You were recently discharged from the hospital.
- You have experienced multiple falls and in increase emergency room visits.
- Significant fatigue, pain, shortness of breath, or poor balance.
- Recent surgery, fractures, Parkinson's, CHF, MS, or blindness.
- Severe post-stroke weakness or high fall risk.
- Coexisting medical conditions contributing to decline.
